Job summary

Ethos is seeking a compassionate RN Hospice Case Manager to coordinate and oversee patient care in the Kansas City area. You will develop individualized plans, perform clinical assessments, and manage symptoms in collaboration with the interdisciplinary team.

This role requires a valid MO RN license, CPR certification, and hospice experience. You will educate families, document care accurately, and ensure timely communication with physicians and leadership to support patients and their loved

Qualifications

Current Registered Nurse license
Current driver's license
Current CPR certification
Two years ofRegistered Nurse experience in a clinical care setting
One year of full-time hospice experience
Reliable transportation and valid automobile liability insurance

Responsibilities

Coordinate and oversee all aspects of assigned patients' hospice care
Develop and implement individualized plans of care with the interdisciplinary team
Perform head-to-toe assessments, monitor symptoms, and manage medications
Educate patients and families regarding condition, needs, and care
Communicate patient progress with clinical leadership, physicians, and the care team
Complete all required documentation accurately and on time
