Qualifications
- Current Registered Nurse license valid for practice in Missouri.
- Current driver's license.
- Two years of RN experience in a clinical setting.
- One year of full-time hospice experience preferred.
- Reliable transportation and automobile liability insurance.
- Ability to work independently and make sound clinical decisions.
Responsibilities
- Coordinate and oversee all aspects of assigned patients' hospice care.
- Develop and implement individualized plans of care with the interdisciplinary team.
- Perform head-to-toe assessments, monitor symptoms, and manage medications.
- Educate patients and families regarding condition, needs, and care.
- Communicate patient progress with clinical leadership, physicians, and the care team.
- Complete all required documentation accurately and on time.
