Job summary

Phoenix Home Care and Hospice is seeking a Full-Time RN Case Manager to deliver compassionate nursing care to clients with terminal diagnoses in Kansas City, MO and surrounding areas. You will collaborate with an interdisciplinary team to develop and implement patient/family centered plans of care, guiding goals of care discussions and healthcare decisions to improve quality of life.

Qualifications

• Missouri or Compact RN license is required.
• Valid driver's license and auto liability insurance required.

Responsibilities

• Provide education to patients and family members regarding the process, plan of care, and healthcare decisions.
• Consult with the attending physician and Hospice Medical Director regarding changes in patient condition and plan of care.
• Coordinate hospice care with other team members and with patients/family to ensure needs are met.
• Conduct initial and ongoing comprehensive assessments and evaluations for hospice care.
