Job summary

Apploi is seeking a transportation coordinator/nurse aide support to organize and provide timely transportation for residents to medical appointments and related facilities. The role involves driving responsibilities, reporting on resident status, and ensuring safety and proper documentation throughout transit.

The position requires a valid state driver’s license, state nurse aide certification, and BLS/CPR.

Qualifications

• Valid driver’s license for the applying state.
• State-approved Nurse Aide Training and Competency Evaluation Program; name must appear on State Registry as Nurse Aide in good standing.
• Current BLS/CPR certification

Responsibilities

• Organizes transportation of residents to appointments based on priority of needs.
• Transports residents to inpatient/outpatient programs and to facilities and residences.
• Develops driving schedule according to residents’ medical appointments and program destinations.
• Reports and assesses resident behavior or condition during transportation; notes abnormal findings to supervisor.
• Transports medical records for signatures to medical offices.
• Maintains safety policies and clean, well-maintained vehicle; logs mileage and performs routine maintenance.
• Assists clinical staff with safe lifting and transfers as ordered by physicians or supervisor.
• Establishes communications with outside facilities to ensure services are scheduled appropriately.
• Reports scheduling changes promptly to DON and nursing/ancillary staff; follows up as needed.
• Provides adequate hydration to residents at regular intervals per individual needs.
• Protects residents’ personal property at all times.
• Other duties may be assigned with or without notice.
