Job summary

PACS is seeking a dedicated caregiver to provide routine daily care to residents inVista, CA. You will follow each resident’s assessment and care plan, support meal and daily activities, and maintain a calm, supportive environment during your shift.

Qualifications

• Must possess, as a minimum, a high school education or its equivalent.
• Ability to read, analyze and follow general procedures and care plans.
• Strong communication and interpersonal skills for resident interaction.

Responsibilities

• Provide routine daily care and services per resident care plan.
• Assist with ALDS, meals, mobility, and personal care.
• Document notes, reports and incidents accurately and timely.
• Maintain resident safety and confidentiality of protected health information.
• Report changes in residents' conditions to nursing staff and supervisors.
• Escort residents and help with transfers between rooms.
• Ensure rooms are clean, beds made, and supplies stocked.

Education
