Job summary

PACS is seeking a Certified Nursing Assistant to provide routine daily nursing care per care plans. You will identify residents, document observations, and support daily living activities to ensure comfort and safety.

The role requires strict adherence to policies, confidentiality, and timely reporting of changes in residents' conditions. The position is on-site, with responsibilities across shift handoffs and resident support, focusing on dignity, safety, and high-quality care in a post-acute

Qualifications

• Must possess, as a minimum, a 10th grade education.
• Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, and technical procedures.
• Ability to write reports, business correspondence, and procedure manuals; ability to present information to managers and employees.

Responsibilities

• Identify residents before administering treatments using wristband or photo ID.
• Record entries on flow sheets, notes, charts in a descriptive manner.
• Report changes in resident condition to Nurse Supervisor/Charge Nurse as soon as practical.
• Assist with daily resident care needs and ensure beds and rooms are prepared for new admissions and discharges.
• Maintain confidentiality of resident care information and report unauthorized disclosures.

Education

Licensed Certified Nursing Assistant (CNA)

CPR certification preferred
