Job summary

PACS is seeking a licensed Certified Nursing Assistant to provide routine daily nursing care to residents in a post-acute facility. You will identify residents, document care, and monitor changes in health status while collaborating with nurses and staff.

Responsibilities include assisting with activities of daily living, ensuring comfort, and maintaining a safe, respectful environment for residents across shifts.

Qualifications

• Must possess, as a minimum, a 10th grade education.
• Licensed Certified Nursing Assistant (CNA) per state law.
• CPR certification preferred.

Responsibilities

• Provide routine daily nursing care and services per resident plan.
• Identify residents before performing treatments or meals with ID wristband or photo card.
• Report changes in residents' condition to Nurse Supervisor/Charge Nurse promptly.
• Document entries accurately on flow sheets, charts, and notes.
• Maintain resident confidentiality and report any unauthorized disclosures.
• Assist residents with daily activities, meals, and personal care needs.
