Job summary

PACS is seeking a dedicated caregiver to provide routine daily care and services per each resident’s assessment and care plan. The role involves using PCC, documenting changes, and coordinating with nursing staff to maintain a safe, comfortable environment for residents.

The position requires a high school diploma or equivalent and proficiency with Microsoft Office. You will work closely with Health and Wellness staff to ensure high-quality resident care and timely reporting of any concerns.

Qualifications

• High school education or equivalent required.
• Ability to read, analyze, and interpret general business materials.
• Ability to write reports, business correspondence, and procedure manuals.
• Proficiency in Microsoft Office tools.

Responsibilities

• Provide routine daily care and services per resident care plan.
• Use PCC to ensure correct resident information.
• Report changes in residents’ condition to Health and Wellness Director.
• Record entries on notes, SPAs, and incident reports in an informational manner.
• Maintain confidentiality of resident care information and report disclosures.
• Greet residents and escort them to their room.
• Assist residents with daily functions (dental, bathing, dressing).
• Make beds and keep rooms ready for residents.
