Job summary

Great Lakes Management is seeking a Resident Assistant in Bloomington, MN to provide hands-on care and emotional support to residents in a long-term care setting. The role requires effective communication with residents, families, and the care team, along with adherence to individualized service agreements.

Responsibilities include assisting with ADLs, supporting residents’ independence, and documenting daily activities while collaborating with nursing staff to ensure quality care for residents.

Qualifications

• Six months of experience in long-term care or assisted living.
• CNA/HHA certification preferred.
• Ability to communicate with residents, families, and the care team.

Responsibilities

• Assist residents with ADLs (Activities of Daily Living) as indicated in the service plan.
• Help each resident reach and maintain the highest level of independence.
• Provide residents with physical and emotional support.
• Assist with treatments as delegated by the Registered Nurse per the service agreement.
• Observe residents and report changes to the case manager.
• Interact with residents to provide reassurance, engagement, and redirection as appropriate.
• Complete all required documentation daily and maintain the service schedule.
