Job summary

Great Lakes Management is seeking a dedicated Resident Assistant in Minnetonka, MN. You will provide hands-on care, support residents emotionally and physically, and assist in dining and activities to enhance quality of life.

Qualifications include at least six months in long-term care or assisted living, and preferred CNA/HHA/RNA credentials. You will collaborate with families, notify the case manager of changes, and complete daily documentation to ensure excellent resident care.

Qualifications

• Six months of experience in long term care or assisted living residence (Preferred)
• Registered as a Nursing Assistant, Home Health Aid or Certified Nursing Assistant (Preferred)
• Ability to make responsible choices and decisions and act in the resident’s best interests.

Responsibilities

• Assist residents with ADLs (Activities of Daily Living) as indicated on the individualized service agreement
• Help residents reach and maintain the highest level of performance and independence
• Provide residents with both physical and emotional support
• Assist with treatments as delegated by the Registered Nurse per the resident service agreement
• Observe clients and report changes in physical and emotional condition to the case manager
• Interact with residents to provide reassurance, engagement, and redirection as appropriate
• Complete all required documentation daily, service schedule, flow sheet, communication book, etc.
