Job summary

Great Lakes Management in Bloomington, MN is seeking a Resident Assistant to provide hands-on care to residents as outlined in each service agreement. You will communicate with residents and families and notify the case manager of changes in condition.

You will assist in the dining room, participate in activities and outings, and help residents with ADLs to support independence and wellbeing. Requirements include six months of experience in long-term care or assisted living and certification as

Qualifications

• Six months of experience in long term care or assisted living residence (Preferred).
• Registered as a Nursing Assistant, Home Health Aid or Certified Nursing Assistant (Preferred).
• Ability to make responsible choices and decisions and act in the resident’s best interests.

Responsibilities

• Assist residents with ADL’s as indicated on the individualized service agreement.
• Help each resident to reach and maintain the highest level of performance and independence.
• Provide residents with both physical and emotional support.
• Assist with treatments as delegated by the Registered Nurse per the resident service agreement
• The Resident Assistant observes clients and reports changes in physical and emotional condition such as change in attitude, loss of appetite, weight, etc. to the casemanager.
• Interact with residents to provide reassurance, engagement, and redirection as appropriate.
• Complete all required documentation daily, service schedule, flow sheet, communication book, etc.

Skills

Experience in long-term care

Caregiving certification preferred

Decision-making in resident care

Education

Nursing Assistant Certification (CNA/HHA/NA)
