Job summary

San Antonio West Nursing and Rehabilitation is seeking a dedicated Certified Nursing Assistant to provide routine daily nursing care aligned with each resident’s care plan. You will help residents with daily activities, monitor vital signs, and ensure comfort and safety, under supervision.

The role requires reporting changes to the Nurse Supervisor, maintaining accurate records, and protecting resident privacy. CPR is preferred, and state CNA licensing is required.

Qualifications

• Education and/or Experience: Must possess, as a minimum, a 10th grade education.
• Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
• Certificates, Licenses, Registrations: Must be a licensed Certified Nursing Assistant in accordance with laws of the state. CPR Preferred.

Responsibilities

• Provide routine daily nursing care and services per resident care plans.
• Identify residents before treatments and meals using wristbands or photo cards.
• Record entries accurately in flow sheets, notes, and charts.
• Report changes in residents' condition to Nurse Supervisor/Charge Nurse.
• Maintain confidentiality of resident information and report any unauthorized disclosures.
