Job summary

Norwood HealthCare in Norwood, MA is seeking a compassionate Physical Therapist to join our rehabilitation team in a skilled nursing environment. You will provide rehabilitative therapy to patients referred for physical, cognitive, and ADL functioning disorders, with a focus on recovery and returning to prior levels of independence.

The ideal candidate holds a Bachelor's degree or higher in Physical Therapy, has 1+ year experience in a skilled nursing facility, and is committed to evidence-based

• Bachelor's degree or higher in Physical Therapy or related field.
• 1+ years of experience in a skilled nursing facility environment.

Responsibilities

• Screen/evaluate patients with physical, cognitive and/or ADL disorders and develop a Plan of Care.
• Provide rehabilitative, skilled, and medically necessary therapy per standards.
• Administer procedures including ADL retraining, therapeutic activities and exercises.
• Assess progress and adjust treatments in the clinical record.
• Document outcomes and coordinate with families and care teams.
