Job summary

Trinity Healthcare Talent Services is seeking a dedicated Physical Therapy Assistant to join a skilled nursing facility in New Prague, Minnesota. In this full-time role, you will work with a licensed Physical Therapist to deliver therapeutic services, help residents regain strength and mobility, and document progress.

Qualifications include an Associate's degree, state licensure eligibility, and strong communication and teamwork.

Qualifications

• Associate's degree from an accredited PTA program.
• Current state license or eligibility for licensure as PTA.
• Previous experience in skilled nursing or rehab setting preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate and patient-centered approach to care.
• Physical ability to assist residents with mobility and therapeutic exercises.

Responsibilities

• Assist the licensed Physical Therapist in evaluating and treating residents according to their therapy plans.
• Implement therapeutic exercises to improve strength, ROM, and function.
• Monitor and document residents' progress and communicate changes to the PT.
• Instruct and educate residents and families on home exercises.
• Collaborate with other healthcare professionals to ensure coordinated care.
• Maintain accurate documentation of therapy services.
