Job summary

Trinity Healthcare Talent Services, LLC is seeking a dedicated Physical Therapy Assistant to join our skilled nursing facility in New Prague, Minnesota. In this full-time role, you will work closely with a licensed Physical Therapist to deliver therapeutic services that help residents regain strength, mobility, and independence.

You will assist with evaluations, perform exercises, monitor progress, educate residents and families, and collaborate with the care team.

Qualifications

• Associate's degree from an accredited Physical Therapy Assistant program.
• Current state license or eligibility for state licensure as a Physical Therapy Assistant.
• Experience in a skilled nursing facility or rehabilitation setting preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate and patient-centered approach to care.
• Physical ability to assist residents with mobility and perform therapeutic exercises.

Responsibilities

• Assist the licensed Physical Therapist in evaluating and treating residents according to their individualized therapy plans
• Implement therapeutic exercises and activities to improve residents' strength, range of motion, and function
• Monitor and document residents' progress and communicate with the Physical Therapist about any changes or concerns
• Instruct and educate residents and their families on therapeutic exercises and techniques to continue at home
• Collaborate with other healthcare professionals to ensure comprehensive and coordinated care for residents
• Maintain accurate and up-to-date documentation of all therapy services provided
