Job summary

Talented Medical Solutions seeks a compassionate Physical Therapist (PT) to deliver high-quality rehabilitation in a healthcare setting. You will evaluate patients, design individualized treatment plans, and apply therapeutic interventions to restore mobility and function.

Requires an active PT license in the state, a Doctor of Physical Therapy (DPT) or equivalent, and BLS/CPR certification. Experience in acute care, outpatient, inpatient rehab, skilled nursing, or home health is preferred;

Qualifications

• Active PT license in the state of practice.
• Current BLS/CPR certification.
• DPT degree or equivalent as required by state regulations.
• Experience in acute care, outpatient, inpatient rehabilitation, SNF, home health or specialty settings.
• Strong clinical assessment, communication, documentation, and patient education skills.
• Ability to work independently and within an interdisciplinary team.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and referrals.
• Provide therapeutic interventions to improve mobility, strength, balance, and functional independence.
• Monitor progress, modify plans, and document evaluations, treatments, and outcomes.
• Educate patients, families, and caregivers on plans, home exercise programs, and discharge recommendations.
• Collaborate with physicians and interdisciplinary teams to coordinate care.
• Maintain accurate clinical documentation in line with policies and regulations.
• Ensure patient safety, infection prevention, and quality assurance standards.
• Treat patients across assigned care settings and age groups.
• Perform other job-related duties as assigned.
