Job summary

Talented Medical Solutions in Waldorf, MD is seeking a compassionate Physical Therapist (PT) to provide patient-centered rehabilitation in our healthcare setting. You will evaluate patients, develop individualized treatment plans, implement interventions, and monitor progress to restore mobility, reduce pain, and improve function.

Collaborate with physicians, nurses, and an interdisciplinary team to ensure safe, evidence-based care.

Qualifications

• Active PT license in the state of practice.
• DPT degree or equivalent per state requirements.
• BLS/CPR certification as required by facility policy.
• Experience in acute care, outpatient, inpatient rehab, or related settings preferred.
• Strong communication and documentation skills.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and physician referrals.
• Provide therapeutic interventions to improve mobility, strength, balance, flexibility, coordination, pain management, and functional independence.
• Monitor patient progress, modify treatment plans as appropriate, and document all evaluations, treatments, and outcomes accurately.
• Educate patients, families, and caregivers regarding treatment plans, home exercise programs, injury prevention, and discharge recommendations.
• Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care and achieve optimal rehabilitation outcomes.
• Maintain accurate and timely clinical documentation in accordance with facility policies and regulatory requirements.
• Ensure compliance with patient safety, infection prevention, and quality assurance standards.
• Treat patients across assigned care settings and age groups as required by the facility.
• Perform other job-related duties as assigned.
