Qualifications

• Licensed Physical Therapist in the state of practice.
• BLS certification preferred; required within 30 days for some locations.
• Minimum 2 years of experience in a similar setting.

Responsibilities

• Evaluates patients, gathers information, and develops treatment plans with goals.
• Provides direct therapy daily following plan of care and standards.
• Maintains accurate records and progress in EMR with timely co-signatures.
• Maintains productivity while ensuring quality care.
• Supervises PT assistants and supports professional growth.
