Job summary

Qualifications

• High School diploma or GED equivalent.
• Able to read, write and communicate effectively with the client and supervisor.
• Able to use the Care Call IVR system.
• Capable of assisting with activities of daily living and following a care plan with minimal supervision.
• Have a valid driver’s license and reliable transportation.
• At least 18 years of age.
• Passed competency testing or completed a competency program performed by RN or LPN.

Responsibilities

• Support for activities of daily living (eating, bathing, hygiene, continence).
• Home support (cleaning, shopping, errands, safety checks).
• Monitor client condition and report changes; cannot administer medications.
• Follow care plan under RN supervision and participate in assessments as scheduled.
