Job summary

Qualifications

• High school diploma or GED required or equivalent.
• Able to read, write and communicate effectively with clients and supervisor.
• Capable of following a care plan with minimal supervision.
• Must have a valid driver’s license or reliable transportation.
• At least 18 years of age.
• Completed competency testing or training as directed by supervisory RN/LPN.

Responsibilities

• Support for activities of daily living (eating, bathing, hygiene, skincare).
• Meal planning and preparation.
• Assist clients in and out of bed and with ambulation.
• Repositioning to prevent bedsores and improve comfort.
• Monitor client condition and report changes to supervisor.
• Provide home support: cleaning, shopping, errands, home safety.
