Job summary

WelbeHealth is seeking a Personal Care Assistant to deliver hands-on ADL support and participate in care planning at our Modesto, CA day centers. You’ll escort participants, monitor vitals, assist with meals, and ensure safety on the day floor.

Requirements include a high school diploma, CA HHA/CNA preferred, and at least 1 year of experience with seniors. Earn a competitive base plus bonus and comprehensive benefits in a supportive, diverse team.

Qualifications

• High school diploma or equivalent required.
• Certification of Home Health Aide (HHA) or Certified Nursing Assistant in California (CNA) preferred.
• Minimum of one year of experience providing personal care to seniors.
• CA Food Handlers Certification within 30 days of hire.
• CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.

Responsibilities

• Provide hands‑on personal care assistance with activities of daily living (ADLs) to WelbeHealth participants, including escorting, toileting, bathing, dressing, grooming, laundry, and lifting when transferring participants.
• Measure and document participants’ vital signs as instructed.
• Act as a medical escort when needed, assisting participants with boarding and exiting WelbeHealth transportation vehicles.
• Oversee activity on the day center floor to assure participant safety.
• Assist with meal service, ensuring participants receive the correct meals, and assist with feeding when needed.
• Maintain personal care supplies such as incontinence supplies and grooming items.

Education

High school diploma or equivalent

HHA or CNA certification in CA (preferred)
