Andwell Health Partners is seeking a Patient Service Navigator for Hospice in Lewiston, ME. The role is office-based, full-time, and involves coordinating communications, referrals, and documentation to support compassionate care.

You will serve as the first contact for patients and families, manage scheduling for clinical staff, and maintain documentation workflows to ensure efficiency and accuracy. A high school diploma and healthcare scheduling experience are preferred.

Qualifications

• High School diploma required.
• Prior office or healthcare scheduling experience preferred.
• Electronic Medical Records experience a plus.

Responsabilités

• Serve as first point of contact for patients, families, and providers—answer calls, handle referrals, and communicate with patients.
• Manage scheduling of clinical staff visits in a busy office.
• Ensure appropriate assignment of referrals in consultation with supervisors.
• Assist in developing and evaluating team documentation systems to ensure accuracy and timeliness.
• Maintain up-to-date knowledge of insurance forms and chart paperwork requirements.
