Job summary

Axess Family Services, Inc. is seeking a Patient Navigator to provide high-quality customer service to clinic patients and visitors. You will coordinate AxessPointe Community Health Center appointments, perform reception duties, and maintain patient charts in a team-based care environment in Middlefield, Ohio.

The role requires 1–3 years with underserved populations, strong communication, and ability to work evenings or Saturdays. Office equipment proficiency and HIPAA compliance are essential.

Qualifications

• High School diploma or equivalent required.
• 1-3 years’ experience serving underserved populations preferred.
• Ability to operate office equipment (computer, fax, printers, scanners, etc.).
• Excellent oral and written communication skills; professional and respectful demeanor.

Responsibilities

• Provide exemplary customer service to patients and visitors in-person and by phone.
• Answer calls and schedule appointments (in-office and telehealth).
• Register patients, verify insurance, and process paperwork (scanning/faxing).
• Collect co-pays and reconcile funds daily; explain payment plans as needed.
• Complete Telephone Encounters in the system and assign to staff.
• Check patients in/out and coordinate with clinical staff for walk-ins.
• Assist patients with navigating insurance, processing claims, and setting up payment plans.
• Complete insurance eligibility verification.
• Travel between AxessPointe locations as needed and support other duties as assigned.
