Job summary

Thyme Care is seeking a Palliative Care Nurse Navigator to join our care teams, serving members with serious illness and their networks by phone, email, and video. Under the Nurse Team Lead, you will conduct symptom assessments, support members longitudinally, and guide end-of-life discussions when appropriate.

Within three months, you will familiarize yourself with Thyme Care systems and partners, collaborate with nurse leaders and medical directors, and build trusting relationships with

Qualifications

• Active RN license required; NY/IL/MI licensure preferred.
• ADN or BSN degree required; BSN preferred.
• At least 5 years nursing experience; 3 years in outpatient palliative care/home hospice for oncology patients and caregivers.
• Certifications such as CHPN/OCN/AOCN/CCM are optional.

Responsibilities

• Conduct goals and values discussions, advance care planning, and palliative care via phone, email, and video.
• Perform comprehensive symptom assessments and longitudinal support for members with serious illness.
• Engage with members and care networks, address clinical issues and escalate as needed.
• Collaborate with Nurse leaders and Medical Directors to align with protocols and best practices.
