Job summary

Thyme Care Inc. is seeking an Oncology Nurse Navigator to join the care team. You will provide triage, education, and clinical support across the cancer journey, working under the Nurse Team Lead.

Responsibilities include conducting assessments, managing escalations, and coordinating care with clinicians and payers. Role operates on a fixed schedule: 8:30 AM–5:00 PM EST, four weekdays plus Saturday. Requires 5+ years of nursing experience with 3+ in solid tumor oncology and active RN license in

Qualifications

• Active, unrestricted RN license is required.
• 5+ years of nursing experience, including 3 years in solid tumor oncology.
• Oncology certifications such as OCN, AOCN, AOCNS, or CCM required or obtained within 2 years of hire.
• Compact license plus single-state licensure in NY and/or IL preferred.
• BSN degree preferred; ADN acceptable.

Responsibilities

• Conduct comprehensive clinical assessments and oversee member health.
• Triage, support, and education during the cancer journey via phone, email, and video.
• Manage escalations and participate in case conferences with the healthcare team.
• Collaborate with Nurse leaders and Medical Directors to ensure protocol alignment.
• Engage with payers and providers to optimize care delivery and reduce readmissions.
• Coordinate care across screening to survivorship or end-of-life care.
• Provide clinical consults and be available for urgent escalations.
• Adhere to care team policies and documentation standards.
