Job summary

Thyme Care, Inc. is seeking a Palliative Care Nurse Navigator to support members with serious illness and their care networks via phone, email, and video.

You will conduct goals of care conversations, advance care planning, and symptom assessments under the guidance of the Nurse Team Lead. Within your first months, you will collaborate with Nurse leaders and Medical Directors, establish trust with members, manage escalations, participate in case conferences, and help coordinate care and social

Qualifications

• Active, unrestricted RN license required.
• New York, Illinois, or Michigan licensure preferred.
• Nursing degree (ADN/BSN) required; BSN preferred.
• At least 5 years nursing experience, including 3 years in outpatient palliative care and/or home hospice for oncology patients.

Responsibilities

• Conduct goals-of-care discussions and advance care planning.
• Provide palliative care via phone, email, and video.
• Perform comprehensive symptom assessments and longitudinal care.
• Coordinate end-of-life care when appropriate.
• Engage with care network, payers, and providers to optimize care.
