Job summary

Thyme Care, Inc. is seeking a Palliative Care Nurse Navigator to support members with serious illness and their care networks via phone, email, and video.

You will conduct goals of care conversations, advance care planning, and symptom assessments under the guidance of the Nurse Team Lead. YOUR ROLE

As a Thyme Care Palliative Care Nurse Navigator, you'll be a vital clinical resource for our members with serious illness and their care network, conducting conversations about goals and values, advance care planning, and palliative care via phone, email, and video communication. Under the guidance of our Nurse Team Lead, you'll conduct comprehensive symptom assessments, support members with serious illness longitudinally, and facilitate end-of-life care discussions, when appropriate. You will actively engage with members, addressing clinical issues and efficiently managing any escalations that arise.

Within your first three months, you will:

  • Familiarize yourself with Thyme Care systems, tools, technology, and partners.
  • Collaborate closely with Nurse leaders and Medical Directors to ensure alignment with clinical protocols and best practices.
  • Establish trusting relationships with members and their care network, prioritizing empathy and active listening in every interaction.
  • Adhere to Care Team policies, procedures, and documentation standards, contributing to efficient operations and maintaining quality standards.
  • Support members with serious illness and at the end of life, coordinating care and providing clinical support as needed.
  • Participate in case conferences to monitor member progress, provide updates, and collaborate on targeted support plans with the healthcare team.
  • Foster strong partnerships with payers and providers to optimize care delivery and minimize readmissions.
  • Collaborate with non-clinical Care Team members to address social determinants of health needs, such as food resources and transportation access.
  • Be available for urgent clinical escalations and provide clinical consult support as required.
  • Perform other projects and duties as assigned and as related to department business needs and objectives.

  • Qualifications: An active, unrestricted compact Registered Nurse (RN) license is required,in addition toNew York, Illinois and/or Michigan licensure preferred. Degree in Nursing (ADN or BSN) required; BSN preferred. Additionally, you have at least 5 years of nursing experience, including 3 years in outpatient palliative care and/or home hospice taking care of oncology patients and their caregivers.
  • Certifications: It would be exceptional, although not required, that you are a Certified Hospice and Palliative Nurse (CHPN) or have oncology-related certifications such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM).

To ensure sufficient clinical coverage, we ask that our Care Team be prepared to work up to 2 holidays per year, compensated at a 1x hourly rate and a 2.5x holiday rate.

This is a non-exempt, full-time position. The pay rate is non-negotiable at $46.35/hour. Due to contractual limitations regarding access to PHIs, you must be located within the lower 48 United States to perform this role. OCN certification is required at time of hire or a commitment to obtain within 2 years of hire at Thyme Care.

Fluency in Spanish, both written and verbal, is preferred.
