Job summary

TEEMA Solutions Group is seeking a compassionate Occupational Therapist (OT) to evaluate patients, develop individualized treatment plans, and implement evidence-based interventions to improve ADLs, mobility, and independence across care settings. The OT will educate patients and families, Coordinate with interdisciplinary teams, and ensure accurate EMR documentation, while adhering to HIPAA and professional standards.

Previous OT experience is preferred.

Qualifications

• Education: Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program.
• Licensure: Active OT license in the state of practice; BLS certification if required.
• Experience: Prior OT experience preferred across settings and patient populations.

Responsibilities

• Evaluate patients' physical, cognitive, sensory, developmental, and functional abilities.
• Develop individualized treatment plans based on physician orders, patient goals, and clinical findings.
• Implement evidence-based OT interventions to improve ADLs, IADLs, mobility, and functional independence.
• Educate patients, families, and caregivers about treatment plans, home exercise programs, and discharge plans.
• Recommend, fabricate, or train patients in adaptive equipment and assistive devices as appropriate.
• Monitor progress, reassess status, modify plans, and document outcomes in EMR.
• Collaborate with physicians and interdisciplinary teams to coordinate care.
• Maintain compliant clinical documentation in EMR.
• Follow facility policies, HIPAA, and professional standards.
• Participate in discharge planning and quality improvement initiatives.
• Supervise COTAs and rehab aides when applicable.
