Job summary

TEEMA Solutions Group is seeking a compassionate Occupational Therapist (OT) to evaluate, develop, and implement individualized treatment plans that help patients regain or improve functional independence and quality of life. The OT provides evidence-based interventions and collaborates with interdisciplinary teams to support patient goals and discharge planning.

California license is required along with a current BLS certification if required by the facility.

Qualifications

• Current OT license in the state of practice and BLS certification if required by the facility.
• Experience across outpatient, inpatient, acute care, rehab, SNF, home health, pediatrics or school settings.
• Ability to evaluate, plan, implement, and document OT interventions per physician orders.

Responsibilities

• Evaluate patients' physical, cognitive, sensory, developmental, and functional abilities.
• Develop individualized treatment plans based on physician orders and patient goals.
• Implement evidence-based OT interventions to improve ADLs, IADLs, mobility, and cognition.
• Educate patients, families, and caregivers on treatment plans and home programs.
• Recommend or train patients in adaptive equipment and assistive devices as appropriate.
• Monitor progress, reassess status, and document outcomes in the EMR.
• Collaborate with physicians, therapists, nursing staff, and others to coordinate care.
• Ensure compliance with HIPAA, safety standards, and facility policies.
• Participate in discharge planning, quality initiatives, and interdisciplinary meetings.
• Supervise COTAs, aides, students, or support personnel as permitted by state regulations.
• Perform other job-related duties as assigned.
