Job summary

TEEMA Solutions Group is actively seeking a compassionate Occupational Therapist (OT) to evaluate patients and develop individualized treatment plans that enhance functional independence and quality of life. The OT will implement evidence-based interventions and collaborate with interdisciplinary teams while maintaining accurate clinical documentation.

Responsibilities include educating patients and caregivers, supervising clinical staff when applicable, and contributing to discharge planning

Qualifications

• Education: OT degree from an accredited program; licensure in the practicing state.
• Current OT license; Basic Life Support (BLS) if required by facility.
• Experience preferred across outpatient, inpatient, acute care, rehab, SNF, home health, pediatrics, geriatrics, or school-based therapy.

Responsibilities

• Evaluate patients' physical, cognitive, and functional abilities.
• Develop individualized treatment plans based on physician orders and patient goals.
• Implement evidence-based OT interventions to improve ADLs, IADLs, mobility, and cognition.
• Educate patients, families, and caregivers on plans, exercises, and discharge recommendations.
• Supervise assistants or students where applicable and within state regulations.
• Maintain compliant EMR documentation and participate in discharge planning.
