TEEMA Solutions Group is seeking a compassionate Occupational Therapist (OT) to evaluate and implement individualized treatment plans aimed at restoring functional independence. You will deliver evidence-based interventions, educate patients and families, and document outcomes in the EMR across care settings.

The role requires OT licensure in the state of practice and relevant clinical experience; BLS may be required by the facility. Collaborative work with an interdisciplinary team is essential.

Qualifikationen

• Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program.
• Current OT license in the state of practice; BLS certification if required by the facility.
• Experience in outpatient, inpatient, acute care, rehabilitation, SNF, home health, pediatrics, geriatrics, or school-based therapy may be preferred depending on the assignment.

Aufgaben

• Evaluate patients' physical, cognitive, sensory, developmental, and functional abilities.
• Develop individualized treatment plans based on physician orders, patient goals, and clinical findings.
• Implement evidence-based OT interventions to improve ADLs, IADLs, mobility, upper extremity function, coordination, cognition, and functional independence.
• Educate patients, families, and caregivers regarding treatment plans, home exercises, adaptive equipment, safety techniques, and discharge recommendations.
• Recommend, fabricate, or train patients in the use of adaptive equipment, splints, braces, and assistive devices as appropriate.
• Monitor progress, reassess functional status, modify plans, and document outcomes.
• Collaborate with physicians, physical therapists, speech therapists, nursing staff, case managers, educators, and other team members.
• Maintain accurate EMR documentation and comply with HIPAA and professional standards.
• Participate in discharge planning, patient education, quality improvement, and interdisciplinary meetings.
• Supervise COTAs, rehab aides, students, or support personnel where applicable.
