Job summary

Sage Creek Post-Acute, part of Grape Holdings, LLC, is looking for a licensed Certified Nursing Assistant to join our care team. You will provide routine daily nursing care in accordance with each resident’s plan and work under the supervision of the Nurse Supervisor.

You will document observations, assist with meals and mobility, maintain a safe environment, and ensure resident comfort while upholding privacy and quality standards. CPR preferred; valid CNA license required by state.

Qualifications

• Must possess at least a 10th grade education.
• Must be a licensed Certified Nursing Assistant.
• CPR certification preferred.

Responsibilities

• Provide routine daily nursing care per resident care plans.
• Identify residents before providing treatments.
• Record observations and report changes to Nurse Supervisor.
• Maintain resident confidentiality and privacy.
