Job summary

Rockingham County in New Hampshire is seeking a dedicated Medication Care Partner to deliver high quality, person-centered care to residents in a fast-paced setting. You will engage with residents and families, support wellness, and report changes to the care team.

Responsibilities include assisting with meals, hygiene, vital signs, and documentation, while following safety and infection control policies. CPR/BLS certified staff are preferred, and a HS diploma with healthcare experience is

Qualifications

• High School Diploma or GED; and
• Two years related experience in a Health Care setting and/or training; or
• An equivalent combination of education, work and experience will be considered.
• Medication Nursing Assistant (MNA) required.
• CPR/BLS Certified.
• National Caregiver Certification Course (NCCC) welcomed, not required.

Responsibilities

• Deliver high quality, person-centered care in a fast-paced environment.
• Develop authentic relationships with residents and families to support wellness.
• Assist with meals, laundry, showering, vital signs and other care tasks as needed.
• Observe/report changes to the Resident Care Coordinator/Manager and follow safety policies.

Education

High School Diploma or GED

Two years related experience in Health Care

Equival ent combination of education/work

Medication Nursing Assistant (MNA) required
