Responsibilities

• Through relationship development and care team coordination, contributes to patient, practice, and organizational goals.
• Assists patient over the course of their visit: facilitates timely flow, informs delays, updates medication and allergy lists.
• Communicates clearly with patient and team: provides hand-offs between patient and provider.
• Performs medical assisting duties under supervision: vitals, EKGs, phlebotomy, immunizations (for MA trained program graduates).
• Lead conversations to meet population health goals through outreach and health coaching; support data gathering.
• Monitors and maintains medical equipment and supplies; ensure PHI used appropriately.
• Queues prescriptions in EHR and documents tasks for proper billing.
