Responsibilities

• Develops empathetic relationships with patients, families and colleagues to support patient care.
• Assists during visits: facilitates flow, informs delays, reviews visit reason and updates medication/allergy lists.
• Communicates hand-offs clearly between patient and provider.
• Performs basic medical assisting duties: vital signs, height/weight, EKGs, phlebotomy, immunizations (where qualified).
• Leads population health outreach and health coaching to meet quality goals.
• Monitors/maintains medical equipment and supplies, ensuring sterility and proper storage.
• Queues prescriptions in the EHR and documents visit tasks for billing.
• Schedules diagnostic tests and verifies insurance, referrals and pre-certifications.
• Identifies OSHA compliance issues and protects PHI.
