Qualifications

Responsibilities

• Develop empathetic relationships with patients and coordinate care with the team.
• Assist patient through visit; manage flow and communicate delays.
• Provide clear hand-offs and document visit details in the EHR.
• Perform medical assisting duties under supervision (vital signs, phlebotomy, immunizations per MA program).
• Lead population health outreach and health coaching to meet goals.
• Maintain medical equipment, ensure sterility and stock; uphold PHI safety.
• Queue prescriptions, enter orders in EHR, assist with tests/appointments.
• Schedule tests; verify insurance, referrals, prior authorizations.
• Identify OSHA issues and ensure patient safety to prevent harm.
