Job summary

Blue Ridge Care in Virginia is seeking a compassionate Licensed Practical Nurse (LPN) to join our PACE program. This hybrid role delivers nursing services in both clinic and home settings, supporting participants with chronic and complex health needs.

You will participate in the on-call rotation to meet participant needs beyond regular hours. Under supervision of an RN and in collaboration with the interdisciplinary team, you will provide direct care, assist with care coordination, and educate

Qualifications

• Active LPN license in the relevant state.
• Minimum 1 year geriatric or PACE/home health experience.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transport.
• Strong communication and teamwork skills.

Responsibilities

• Provide direct clinical care including meds, wound care, vitals, and injections in clinic and home.
• Support care coordination with Clinical Nurse and IDT.
• Educate participants and families on disease management and adherence.
• Document care in the EHR accurately.
• Maintain infection control practices.
• Participate in IDT meetings and on-call rotation.
• Perform other duties to support participant care.
