Job summary

Neighborhood Healthcare is seeking a licensed vocational nurse (LVN) to provide direct nursing care, patient monitoring, care coordination, and clinical support under the direction of a registered nurse and/or primary care provider. You will work with an interdisciplinary team to promote participant health, safety, independence and quality of life, with a Monday–Friday schedule from 8 a.m.

to 5 p.m. Responsibilities include nursing assessments, vital signs documentation, medication

Qualifications

• Valid California LVN license required and maintained as a condition of employment.
• AHA-approved CPR and First Aid certification required for all PACE employees.
• 1 year of nursing experience preferred.
• Experience with geriatric populations, chronic disease management, home health, long-term care, or PACE programs preferred.

Responsibilities

• Provide direct nursing care to participants in the PACE center, clinic, and/or participant homes as assigned.
• Perform nursing assessments within the LVN scope of practice and report changes in participant condition to the RN and PCP.
• Obtain and document vital signs, weights, blood glucose readings, and other clinical data.
• Administer medications and treatments as prescribed and within scope of practice.
