Job summary

Pacific Coast Post Acute in California seeks a part-time nurse/care coordination professional to develop and implement care plans with residents and families. You will collaborate with physicians, nurses, and payers to ensure high-quality, cost-aware patient care.

The role requires a nursing degree with license (RN preferred; LVN/LPN acceptable) and knowledge of Medicare/Medi-Cal; CCM is a plus. Strong communication and organizational skills are essential.

Qualifications

• Bachelor's Degree in Nursing or Social Work; RN license preferred, LVN/LPN acceptable.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid programs and benefits.
• PCC Knowledge preferred.
• CCM certification a plus.
• Must maintain continuing education/licensing.
• Must remain in good standing with the Department of Public Health, License and Certification Division at all times.

Responsibilities

• Communicate plans of care with residents and family about PT, OT and other treatment protocols.
• Attend and participate in morning meetings to facilitate team communications.
• Answer residents' questions about care, treatment plans and illness progression.
• Monitor and adjust residents' statuses based on changing needs.
• Organize and prioritize daily work by assessing residents' needs.
• Complete required documentation.
• Coordinate care with residents, care providers, facilities Financial services, and third-party payers.
• Oversee admissions and discharge activities.
• Coordinate referrals to and from the facility.
• Maintain professional, respectful interactions with residents, families, staff, and partners.
• Apply accurate charting and timely paperwork.
