Qualifications

• Current LPN/LVN California license required.
• Bachelor's degree in Nursing or Social Work.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid programs.

Responsibilities

• Communicate regularly with residents and families about plans of care and treatment.
• Attend morning meetings to coordinate with the care team.
• Monitor residents' status and adjust care plans as needed.
• Complete documentation in a timely manner and ensure accurate charting.
• Coordinate admissions, discharges and referrals to and from the facility.
