Job summary

McKinley Park Care Center in Sacramento is seeking a skilled Care Coordinator with nursing or social work credentials to develop and oversee comprehensive plans of care for residents.

You will collaborate with families, physicians, and staff, perform clinical assessments, and coordinate admissions and discharges while ensuring compliance with Medicare/Medicaid guidelines and maintaining high-quality, cost-conscious care.

Qualifications

• Bachelor's degree in Nursing or Social Work required.
• RN license preferred; LVN/LPN acceptable.
• Minimum 2 years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid programs.
• CCM certification is a plus.

Responsibilities

• Develop and implement care plans with residents and families.
• Communicate plans of care and treatment protocols.
• Attend morning meetings to coordinate with the team.
• Perform clinical assessments and care coordination.
• Coordinate admissions and discharges.
• Complete required documentation accurately.
• Utilize utilization review to ensure cost-effective care.
• Relate positively with residents, families, staff and professionals.
• Manage time in a high-volume environment with changing priorities.
