Trilogy Health Services is seeking a licensed Practical Nurse (LPN) in Ohio to provide direct clinical care on the unit and supervise nursing assistants. You will monitor residents, respond to changes in condition, administer medications, and document care to ensure quality outcomes.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current, valid CPR certification required.

Responsibilities

• Visit residents on assigned unit daily to observe status and notify nursing leadership of changes.
• Identify resident problems and emergencies and initiate lifesaving measures if needed.
• Provide holistic care within the LPN scope.
• Assist in monitoring medications, supplies, and equipment inventory.
• Perform administrative duties such as forms, evaluations, and charting.
• Report occupational exposures to blood, body fluids and hazardous materials.
