Job summary

Cascadia EE LLC is seeking a qualified LPN to provide patient- and family-centered nursing care under the direction of an RN in our Oregon facility.

The role includes direct care, rounds with physicians, care planning, medication administration, and meticulous documentation to ensure safety and quality for elderly residents. CPR certification is required and prior long-term care experience is preferred.

Qualifications

• Graduate of accredited LPN program required.
• Current valid LPN license in the state employed.
• Valid CPR certification required.
• Six months experience in long-term care preferred.

Responsibilities

• Provide direct care and/or assist with care per scope of practice and competency.
• Initiate SBAR and collaborate with RN for assessment and recommendations.
• Assist with rounds with physicians and other team members.
• Coordinate care activities and communicate changes to team and families.
• Administer medications and document responses per policy and regulations.
• Maintain accurate narcotic records and prepare/organize medications.
• Document care and resident status promptly in the medical record.
