Job summary

Cascadia EE LLC is seeking a licensed practical nurse (LPN) to provide patient- and family-centered care under an RN's direction in a long-term care setting. The role includes direct care, medication administration, and documentation to ensure safety and comfort for residents.

Responsibilities include rounds with physicians, care planning participation, and SBAR communication to coordinate care with the health team and residents' families.

Qualifications

• Graduate of accredited LPN School of Nursing.
• Current valid LPN license in the state employed.
• Valid CPR certification.
• Six months experience in a long-term care environment preferred.

Responsibilities

• Provides direct care and/or assists with care as directed and consistent with scope of practice.
• Initiates SBAR communication and collaborates with the RN for assessment and recommendations.
• Assists with medication administration and documentation per policy.
• Documents resident status and care in the clinical record.
• Participates in care planning and communicates changes to the team and family.
• Ensures punctuality and regular attendance for assigned shifts.
