Job summary

Qualifications

• Graduate of accredited LPN School of Nursing.
• Current valid LPN license in the state employed.
• Valid CPR certification.
• Six months experience in a long-term care environment preferred.

Responsibilities

• Provide direct care under supervision and as directed within scope.
• Participate in resident evaluations and care planning activities.
• Administer medications and monitor resident responses.
• Document care and observations promptly in the chart.
• Collaborate with RN and health care team for assessments and recommendations.
• Assist with emergency situations and maintain medication records.
