Job summary

Qualifications

• Graduate of accredited LPN School of Nursing.
• Current valid LPN license in the state employed.
• Valid CPR certification.
• Six months experience in a long-term care environment preferred.

Responsibilities

• Provide direct care under RN direction and initiate SBAR, coordinating with physicians and team.
• Participate in resident evaluations and care planning, revise plans as needed and communicate changes to family.
• Administer medications and document administration; maintain narcotic records and safety practices.
• Transcribe and implement physician orders, communicate with physicians, and assist with emergencies including CPR.
• Complete admission paperwork and assist with discharge processes.
• Maintain accurate clinical records and ensure timely shift change supplies.
