Job summary

Hebrew SeniorLife is seeking a Licensed Practical Nurse (LPN) to collaborate with a Registered Nurse in delivering safe, person-centered care that enhances residents’ quality of life.

You will work with an interdisciplinary team, document care in the electronic medical record, support autonomy and patient choice, delegate to nursing assistants, and participate in rounds and performance improvement activities. This on-site position requires MA LPN licensure and excellent communication skills.

Qualifications

• A graduate of an approved school of nursing is required.
• Current Massachusetts license as an LPN in good standing.
• Previous experience preferred.
• Excellent English language skills, verbal and written.
• Computer literacy required.
• Professional, proactive, collaborative and patient-focused.

Responsibilities

• Ensure care delivered within safe nursing practice and established standards.
• Collaborate with multidisciplinary team to meet patient needs and monitor outcomes.
• Document care in the electronic medical records and generate required reports.
• Delegate to nursing assistants and supervise care delivery.
• Participate in care planning, rounds, and quality improvement activities.
• May assume charge nurse responsibilities with RN oversight.
