Job summary

Trilogy Health Services, LLC in Kendallville, IN is seeking a Licensed Practical Nurse (LPN) to deliver direct, quality clinical care and supervise nursing assistants on the unit.

You will observe residents daily, identify changes in condition, and provide holistic care within the LPN scope. Responsibilities include medication/inventory monitoring, charting, and safety reporting to maintain resident well‑being.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current, valid CPR certification required.

Responsibilities

• Visit residents daily to assess health status and notify changes to charge nurse or physician.
• Identify problems and initiate lifesaving measures in absence of a physician.
• Provide holistic care within LPN scope.
• Monitor medications, supplies, and equipment to ensure availability.
• Complete medical forms, reports, and charting as needed.
• Report occupational exposures per facility policies.
