Job summary

CVS Health seeks a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to deliver high-quality patient care in a community setting. You will assess needs, administer treatments, and collaborate with interdisciplinary teams to support better outcomes.

Responsibilities include monitoring vitals, documenting responses, conducting medication reviews, and guiding patients on disease management and self-care. Strong communication and EMR proficiency (Epic preferred) are essential.

Qualifications

• Active and in good standing LVN or LPN license.
• NCLEX-PN passed.
• Knowledge of medical terminology and clinical documentation standards.
• Experience with phlebotomy, vaccine administration, and point-of-care testing.
• Excellent communication and organizational skills.
• Experience in clinical settings with diverse cultural dynamics among staff and patients.
• Proficiency with EMR systems; Epic experience preferred.

Responsibilities

• Administer medications (oral, injectable, IV, topical).
• Evaluate patient conditions and monitor treatment responses.
• Monitor vital signs and document abnormalities.
• Perform routine health assessments and ensure well-being.
• Assist patients in accessing healthcare and community resources.
• Conduct medication reviews and assess drug interactions.
• Develop and administer mental health assessments as needed.
• Document patient history accurately in electronic records.
• Communicate effectively with patients, families, and clinicians.
