Job summary

CVS Health is hiring an LPN/LVN to deliver high‑quality patient care in a community clinic setting. You will assess patients, administer medications, document in the EMR, and collaborate with a multidisciplinary team to support better health outcomes.

Qualifications include an active LVN/LPN license, NCLEX-PN, phlebotomy and vaccination experience, and strong communication skills. This is a full‑time, on‑site role with comprehensive benefits and a pay range reflective of local market.

Qualifications

Active and in good standing state LVN or LPN license.
Successful completion of the NCLEX-PN (National Council Licensure Examination for Practical Nurses).
Knowledge of medical terminology and clinical documentation standards.
Experience with phlebotomy, vaccine administration, and point‑of‑care testing.
Excellent communication and organizational skills.
Experience working in a clinical setting with diverse cultural dynamics among staff and patients.
Proven ability to manage sensitive information ethically and responsibly.
Strong problem‑solving, critical thinking, and decision‑making skills.
Proficiency with electronic medical record (EMR) systems; Epic experience preferred.

Responsibilities

Administer oral, injectable, IV, and topical medications.
Evaluate complex patient conditions, monitor pain levels, and document treatment responses.
Apply advanced knowledge of vital sign monitoring to identify and record abnormalities.
Perform routine assessments to monitor patient health and well‑being.
Manage patient access to healthcare and connect individuals with community resources.
Conduct medication reviews to identify potential side effects and drug interactions.
Develop and administer mental health assessments to screen for depression and assess mood.
Document patient medical history using electronic health records.
Interact with patients, families and other healthcare professionals.
Keep a clean, organized clinic at all times.
